The Mount Washington Valley Rural Health Primary Care team has been serving the community since 1978. We are dedicated to the health needs of your entire family. We are a multidisciplinary group of family practitioners with sub-specialists in behavioral health and pediatrics. We offer an array of health services for prevention and treatment of acute and chronic illness for all age groups.

Primary Care Team

We use a team-based primary care model. Your care team includes a physician, an advanced practice provider (either a nurse practitioner or physician assistant), and support team members including registered nurses and health care assistants. Your team you will see you for:

  • Regular checkups and health screenings
  • Diagnosing and providing treatment plans for acute and severe illnesses
  • Providing recommended immunizations based on age or other circumstances (such as travel)
  • Coordinating care with other providers within Memorial Hospital, the MaineHealth system, or elsewhere

When you call to schedule a visit (or schedule online using MyChart) you will be communicating with a team that already knows you and knows your health history.

Family Medicine

Our integrated family medicine team provides both inpatient and outpatient pediatric care.

NCQA Recognized Patient Centered Medical Home

We received NCQA Patient-Centered Medical Home (PCMH) and diabetes care recognition for using evidence-based, patient-centered processes that focus on highly coordinated care and longā€term, participative relationships. 

Integrated Behavioral Health Care

Onsite licensed clinical social worker works ensures the best care coordination between your primary care doctor and your behavioral health specialist
